Voluntary Acknowledgement of Paternity

Paternity may be established without going to court. If the child was born out of wedlock, you and the child's mother may sign a voluntary Acknowledgment of Paternity form. This form may not be used if the mother was married. It is important that you're sure that you're the biological father before signing this form, as it is a legal document. Before you sign it, you should read the form and disclosure statement carefully. After a limited timeframe, you waive your rights to a court hearing and will be responsible for child support until the child is 21 years of age.
